First Visit

Thank you so much for choosing Haven Dental as your dental home. We are looking forward to meeting you!  We ask that you arrive a few minutes prior to your appointment time so that we can gather your medical history. 

Your initial appointment will consist of radiographs and an examination followed by a review of your treatment needs. We will work with you to formulate a treatment plan that aligns with your values. 

If you have insurance please bring the policy holder’s social security number and date of birth. We will do a complimentary insurance verification and will work with the insurance company on your behalf for any treatment needs you have.
